Many Epizoanthus taxa in the Mediterranean are doubtful. Some studies considers that Epizoanthus tergestinus could be a synonym of E. univittatus (Lorenz, 1860).

This species is considered endemic to the Mediterranean Sea, however the only confirmed records are from Croatia in the Adriatic Sea, and the species is considered threatened there. However the full distribution of the species is not understood, there is a lack of information on its threats, ecology and population, and further taxonomic study is required. The species is listed as Data Deficient in view of the absence of information on its population size and trend, status, and threats.

Range Description:Epizoanthus tergestinus is present in the Mediterranean Sea although its distribution is unknown and Gili et al. (1987) consider it a doubtful species. It is recorded as present in Croatia.
